By accessing Adena Health's Affiliation Verification website, you acknowledge and attest to each of the following: 1. You are accessing the private network of Adena Health for a very limited purpose and of limited duration; 2. Your organization is a medical facility or authorized agent that will utilize the information provided by this website for protected peer review purposes only; 3. You have been specifically approved by your organization to access this Adena Health website; 4. You have not and will not share your login credentials (username and password) with anyone; 5. You have on file a valid and current Release of Information signed within the last six months by the physician/advanced practice provider whose information you seek to access and said Release of Information grants you permission to obtain access to certain of his/her information regarding his/her affiliation and privileges from Adena Health; AND 6.
